Steps to Obtaining a Medical Cannabis Card in Utah
1. Check to see if you qualify. Click the 'Do I Qualify?' link above for more information.

2. Schedule an appointment with a Qualified Medical Provider (QMP)
When scheduling an appointment with your Medical Marijuana of SLC provider you MUST complete all three of the following, links provided at time of booking.
a. Consent form with electronic signature
b. Create your UtahID account
c. Create your Electronic Verification System (EVS) profile

Once your UtahID and EVS profile is created you are now ‘Awaiting Certification’

3. During your appointment your QMP will access your EVS profile and provide certification
​
4. After certification, the patient must pay the medical cannabis card application fee via EVS (Initial & Renewal: $15)

Your profile is then submitted to the Department of Health and Human Services (DHHS) for Approval
​
5. DHHS reviews medical cannabis card application. For adult patients ages 21 and older with a qualifying condition, the application review process will be completed 15 days or less from the date the application is submitted.

6. If approved you will then receive a medical cannabis card. DHHS will issue the card and email the patient a copy of their medical cannabis card. The patient can then save the medical cannabis card on their smartphone or print it out.

7. Before the first purchase of medical cannabis a patient will need to consult with a medical cannabis pharmacist. Some pharmacies require an appointment made in advance for this consultation.

8. Purchase medical cannabis from a medical cannabis pharmacy. Once a medical cannabis card is issued by the DHHS, a patient is ready to purchase a product from a medical cannabis pharmacy. To purchase medical cannabis, a patient must bring their medical cannabis card and a valid form of photo identification, such as a driver’s license, with them to the pharmacy.

9. Your medical cannabis card expires 1 year after first application. Cards can be renewed with a telehealth/video visit with a QMP.
